Nurse Director of Case Management

We are seeking an experience and dedicated Nurse Director of Case Management for our community hospital in the Miami, Florida area.

Job Summary:

The Director of Case Management will direct and facilitate all processes that relate to the management and utilization of clinical resources including case management, social work services and home care planning. Responsibilities include participation in the negotiation process of managed care contracting; develops systems to effectively manage managed care contracts within the organization; provides services that meet medical, administrative, legal, ethical, and regulatory and hospital requirements; develops, monitors and maintains data collection systems to be used by senior management for hospital strategic planning. The position also requires the ability to function in a highly collaborative manner with the Controller and COO in developing and implementing strategic plans for successful managed care contracting; with the VP Quality in facilitating the data collection process leading to outcomes management; and to have a high level interface with nursing administration in coordinating the overall case management program.

Job Responsibilities:

  • Practices autonomous, evidence-based care in a collaborative environment
  • Provides culturally sensitive care for all patients specific to the population served
  • Coordinates all care with regard to the wholeness of the patient while providing individualized care
  • Assure medical necessity, appropriate level of care/services, and timely interventions are established for all patients
  • Provide leadership and direction for the department under the guidance of the CFO
  • Oversee financial management of the department
  • Manage department personnel
  • Establish and maintain effective communication
  • Direct the performance improvement activities of the department
  • Maintain a safe department work environment
  • Plan, organize and administer work in the Case Management Department by establishing services, policies and procedures in accordance with hospital policies and standard practices of professional organizations within budget and productivity standards to ensure that continuous, accurate, timely and cost effective services are provided.
  • Interpret organizational and departmental policies for staff, analyze and resolve conflicts and initiate plans to motivate staff and achieve work goals.
  • Assure that all staff is properly trained in and oriented to the work environment and have the necessary equipment and supplies to perform their job to attain high quality results and a cohesive work environment. Ensures staff is competent in use of equipment and/or supplies.
  • Hires staff, completes annual performance evaluations and coaches staff to improve work performance and provide high quality service.
  • Attend appropriate hospital committee meetings, conducts monthly departmental meetings, in-services as needed.

Qualifications:

  • Required: Bachelor’s Degree;  Registered Nurse with an active license to practice in the state of Florida
  • Preferred: Master’s Degree; Certification in Case Management a plus

Experience:

  • Minimum five to seven years case management experience
  • Three years or more of progressive Leadership experience
