Opportunity:

The Restorative Nurse will support and utilizes the nursing process to assess, plan, implement and evaluate patient care in our Long-Term Care Facility. Will perform focused interview to identify specific needs. Assess signs and symptoms indicating physiologic and psychological changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Responsibilities:

  • Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
  • Identifies and prioritizes actions based upon patient care requirements.
  • Monitors consistency between patient plan of care and actions of medical staff.
  • Performs interventions according to identified priorities, plan of care, and the facility policies and patient outcome standard.
  • Provides a safe, comfortable and therapeutic patient environment.
  • Initiates and evaluates patient and/or family education
  • Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
  • Document’s patient’s response to nursing and other interventions.
  • Uses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities.
  • Monitors safety and effectiveness of the environment and equipment.
  • Participates in activities to facilitate good interpersonal communications with the chosen facility.

We are seeking LPN’s and RN’s  to work in our Extended Care Facility in Elmhurst, MA.  You must be be skilled in Skilled Care, Long Term Care or Rehab Care to qualify.

*** 2,000 Sign on Bonus ***

Shifts available:

Full Time Hours: 8 hour shifts and 12 hour shifts, 3:00pm to11:00pm and 11:00pmto 7:00am

Part Time Hours: 7:00am – 3:00pm

Differential: $2.00 an hour for 3:00pm to 11:00pm and 11:00pm to 7:00am shifts additional $1.00 an hour for weekends

Overview:

Provide Nursing services to our Skilled Care, Long Term Care and Rehab Residents. Following the guidelines provided by our Corporation. This position reports to the Director of Nursing and may be required to supervise other staff positions.

Essential Functions:

  • Responsible for medication administration for the Assisted Living residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines
  • May be responsible for the admissions of new resident to include all admission assessments and documentation
  • Responsible for completing acute and long term care plans
  • May be responsible for clinical system tracking eg. Wounds or infection control under the direction of the director of nursing
  • Provides general consultation regarding health-related concerns within the community
  • Notifies physicians and/or family members of any change in resident’s health and provides proper documentation
  • Responds quickly to all emergencies
  • Maintains close communication with all departments and staff members
  • Provides educational meetings for residents and staff
  • Understands roll in the Safety & Disaster Plan
  • Maintain confidentiality of residents’ and prospective residents’ information
  • Attends all required training, in-service, and staff meetings
  • Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices
  • Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers
  • Adheres to all policies and procedures
  • Performs other duties as assigned

Qualifications:

  • Must have a Licensed  Practical Nurse or a Registered Nurse in the State of Illinois
